Bespoke vs made-to-measure

The operative distinction
Bespoke drafts a paper pattern for one person, cuts the cloth by hand with inlays, assembles the garment loosely, tries it on, marks the corrections and takes it apart. Two to four fittings, and the pattern is kept and refined.
Made-to-measure starts from an existing block in the nearest size and modifies it (longer sleeve, wider waist, dropped shoulder), usually with one fitting or none.
Both can contain a great deal of hand work, which is why hand work is not the test.
Where the advantage actually lies
In how far your body is from average. A block absorbs moderate adjustment well; asked to accommodate a genuinely asymmetric figure it distorts elsewhere. A drafted pattern has no such ceiling.
If you are close to a standard size, an excellent made-to-measure garment will fit you very well for a fraction of the money, and the remaining difference is construction and finish rather than fit.
The abuse of the word
There is no legal definition of bespoke in most jurisdictions, and the word is applied to services that involve no individual pattern at all. Three questions settle it: is a paper pattern drafted for me alone, is it kept, and will you take the garment apart if the fitting shows a problem?
A real bespoke house answers all three immediately. Anything else is made-to-measure, which is a legitimate product that deserves its own honest name.
